PHYSICIAN/HOSPITAL NETWORK The program utilizes one
of the largest physician networks in the nation. This
nationally recognized network "owns" their physician and
hospital contracts so it cannot lose providers due to
cancellation of a lease agreement. Plus, the network
maintains control of physician education and service levels. The Physician Visit /
Hospital Network is effective and easy to use. To locate
providers in your area, call the toll-free number located on
your membership card. Present your card at the provider's
office and receive immediate savings on services provided.
Since there is no age or medical history restrictions,
anyone can use this benefit.
For hospital visits,
members MUST
pre-register before any procedure(s) are performed (or
within 48 hours of emergency room admittance.) A
representative will coordinate all of the resources and
negotiate with the provider/facility for a contracted

Physician/Hospital Network provides discounts on health care
services at physicians' offices and hospitals. Save an

around-the-clock access to telephone medical consults.
TelaDoc physicians evaluate and diagnose most minor medical
problems and, when appropriate, prescribe medications.
Members can reach a consulting physician from their home,
office, hotel room, or on the road – 24/7/365. Accessing
medical treatment is easy and affordable.
TelaDoc
consulting physicians are all experienced doctors, licensed
to practice medicine in their respective states. They
provide medical diagnosis via review of the patient's
medical history coupled with a telephone consult. To become
a consulting physician, each must undergo a thorough
credentialing process. TelaDoc checks the National Physician
Data Base (NPDB) and the American Medical Association (AMA)
to verify medical licensure, training and education, work

The Proactive Identity Theft Prevention
program will place and maintain fraud alerts on member
credit reports with all major consumer reporting agencies -
Equifax, Experian and TransUnion.
These alerts last
for 90 days, whereupon LifeLock will renew them automatically.
Members receive a copy of each of these reports once
annually from the bureaus so they can ensure the information
creditors are using to judge credit worthiness is true and
accurate. These
credit reports will show the fraud alerts in place in your
behalf. Fraud
alerts require anyone accessing a member’s report to first
call the member prior to making any change in your credit.
Members will also notice a dramatic decrease in the amount
of junk mail and pre-approved credit card offers in the
mail. This support does not mean
